Crynodeb
You will work directly with a small team including an Advance Nurse Practitioner, Community Paramedic and Care Co-ordinators to provide support to patients who are care home residents along with frail older people in their own homes. You will ensure that the PCN and member Practices meet their requirements to provide enhanced health in care homes as well as working with care homes to develop and promote good practice to maximise the health and quality of life of their residents.
