Job Responsibilities Patient Engagement & Communication Dealing with high volume contacts, initiated patients using the online booking system, efficiently and courteously, putting patients at ease and providing clear and concise information. Communicate clearly and empathetically with patients about medication reviews, prescription updates, and safety monitoring. Promote awareness of medicines safety, adherence, and the benefits of regular reviews, especially for those on multiple medications. Prescription Management & Medicines Optimisation Assist in the efficient processing of prescription requests, medication synchronisation, and the tracking of medication changes from hospital discharge summaries or specialist letters, in line with local and practice policies. Act as a point of contact for care homes and residential homes, providing support with prescription queries, medication synchronisation, and minimising medicines wastage Support by helping to resolve prescription related queries, under supervision, while complying with legal and professional requirements Implement and support use of monitored dosage systems and support the district nursing team with medicine optimisation for housebound patients and liaising with community pharmacy staff. Work with the pharmacy team to implement medicines optimisation initiatives and help reduce unnecessary prescribing or medication waste. Liaise with community pharmacies and secondary care providers to resolve prescription queries, shortages, or synchronisation issues. Clinical Support & Project Work Follow practice recall systems to coordinate and schedule appointments for reviews based on clinical risk, frailty, or long-term conditions (e.g., diabetes, asthma, hypertension), with the appropriate clinicians. Support QOF/IIF/CQC/LCS work-streams, by arranging appointments, tests and the follow-up of appointments. Assist in arranging appointments for specific pharmacist-led reviews (e.g. opioid reduction, inhaler technique checks, anticoagulant monitoring). Support practice audits and local medicines optimisation schemes by managing recall lists and maintaining accurate patient records. Facilitate patient referrals to services such as repeat dispensing, new medicine services, pharmacy contraception services, smoking cessation services, or other appropriate interventions, as needed, in line with practice protocols.
