We are committed to delivering high-quality, accessible healthcare to our local community. Our multidisciplinary team includes GPs, nurses, healthcare assistants, and administrative staff, all working collaboratively to provide excellent care. We are part of the Staffordshire and Stoke-on-Trent Integrated Care Board and work closely with local health and social care providers to ensure joined-up, holistic care for our patients. Job Summary We are seeking a highly motivated and experiencedAdvanced Nurse Practitioner (ANP)to join our team. The ANP will work autonomously and collaboratively within our primary care team to provide high-quality, patient-focused care. This role offers an alternative and complementary service to that traditionally provided by GPs, improving access and continuity of care for our patients. The ANP will be responsible for managing a wide range of acute and chronic conditions, conducting home visits, and delivering complete episodes of care. The successful candidate will be a Registered Nurse with advanced clinical training (at degree or masters level), an independent prescribing qualification, and experience in primary care. Key Responsibilities Provide same-day appointments for patients presenting with minor illnesses and injuries. Independently assess, diagnose, treat, and manage patients of all ages within scope of practice. Prescribe medications appropriately and safely within defined clinical remit. Deliver complete episodes of care, including follow-up and safety-netting. Conduct home visits where clinically appropriate. Maintain accurate and timely clinical records in line with practice policies. Work collaboratively with GPs, nurses, and other healthcare professionals to ensure coordinated care. Participate in clinical meetings, audits, and quality improvement initiatives. Support the education and development of other staff where appropriate. Conditions Managed by the ANP The ANP will be expected to manage patients presenting with the following conditions: Abdominal pain Allergies and skin rashes Burns and wounds Chest infections and asthma Coughs, colds, and sinusitis Diarrhoea and vomiting Ear infections and eye problems Eczema and hay fever Fever and headaches Head lice and worms Minor injuries and sprains Sore throats and thrush Urinary tract infections (UTIs) Menstrual bleeding, mastitis, breast lumps Menopause-related concerns Mens health issues (e.g., testicular lumps, prostate concerns) Anxiety, depression, and other common mental health concerns Exclusions When Patients Will Be Referred to Other Clinicians Patients will be referred to a GP or other appropriate clinician in the following cases: Children under 1 year of age Repeat prescriptions Pregnancy-related concerns (ANP unable to prescribe) Womens health issues requiring a vaginal examination Musculoskeletal pain (referred to First Contact Physiotherapist) Person Specification Essential: Registered Nurse with valid NMC registration Masters level qualification in Advanced Clinical Practice or equivalent Independent Prescribing qualification (V300) Proven experience in a primary care or urgent care setting Excellent clinical assessment and diagnostic skills Ability to work autonomously and as part of a multidisciplinary team Strong communication and interpersonal skills Commitment to patient-centred care and continuous professional development Desirable: Experience in conducting home visits Familiarity with EMIS or similar clinical systems Teaching or mentoring experience What We Offer A supportive and collaborative working environment Flexible working hours on Fridays Opportunities for professional development and training NHS pension scheme Access to staff wellbeing resources If you are passionate about delivering high-quality care and want to be part of a dynamic and supportive team, we would love to hear from you.
