Clinical responsibilities To lead the outreach team, reaching and providing clinical care to people who are experiencing homelessness in various contexts around Brighton & Hove and who are struggling to access the surgery; supporting in complex case management of patients with multiple compound needs. To take clinical leadership of patient care across various contexts, prioritising work effectively. To effectively engage patients from a variety of backgrounds. To independently undertake effective comprehensive holistic health assessments for patients including independent assessments of risk. To take time to listen to patients who may have highly distressing personal circumstances and stories to understand their needs. To make appropriate referrals for patients as necessary. To proactively identify the need for mental capacity assessments, safeguarding referrals, challenges to NHS charging decisions within the caseload and take action as required. To clinically advocate and negotiate on behalf of patients with partners, health colleagues and hospital staff. To attend case conferences and multi-disciplinary team meetings as necessary. To work with the wider team and partners to provide opportunities for patients to engage with their health in the community or street. Project Coordination To develop systems and processes to support the delivery of the project. To audit and provide reports on the project when requested. To foster strong partnerships with partner organisations such as CGL, SCFT, Justlife and others. To manage staff associated with this project, providing developmental leadership. To represent the outreach team presenting at conferences and other local local and occasional national forums. Be willing to engage with teaching and training responsibilities for medical/nursing students gaining experience and/or support workers Administrative To maintain robust systems of data collection, audit and review of all clinical activities, liaising with fellow colleagues in the practice and in the local and national health system as needed to ensure good patient care and service is maintained and to satisfy the standards of external regulatory bodies such as the CQC etc. To maintain accurate patient records, updating the practice system as is required for good patient care. Communication Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment. Utilise communication skills to support patients to adhere to prescribed treatment regimens. Personal Responsibilities To maintain your own continuous CPD and ensure a PDP is kept up to date in line with the practice and nursing/clinical team development plans. Recognise and work within own competence and professional code of conduct as regulated by your relevant professional body. To maintain your own annual registration with the relevant professional body and adhere to its Code of Professional Conduct.
