Crynodeb
As a CHC Nurse within Lower Lea Valley ICT, you will be responsible for managing a caseload of patients requiring new CHC assessments and reviews of previous assessments. You will regularly liaise with the CHC team, GP surgeries, ICT nursing and therapy teams and many more professionals to support your assessment and decision making. You will manage your own diary to ensure deadlines are met to complete assessments and relevant paperwork submissions. You will work closely with the ICT staff, to offer your expert support and to discuss any relevant patients with the team. We look forward to hearing from you!
