We are looking for a skilled nurse to join our Rapid Response Team in Maidstone as Team Leader. Your role will involve:
• Conducting comprehensive assessment, planning, implementation, and evaluation of holistic patient care.
• Developing integrated care plans in collaboration with patients to ensure personalized and effective treatment.
• Coordinating the ordering of necessary equipment and facilitate onward referrals as required.
• Serve as a senior practitioner and lead, providing guidance to junior staff in conducting assessments and reassessments of service users and pathways with complex needs within the relevant field.
• Actively contribute as a member of the multi-professional team to deliver high-quality patient care.
• Perform a range of advanced clinical interventions, promptly identifying and addressing any deterioration in patients’ conditions.
• Managing a daily caseload of patients, focusing on those with complex needs.
• Provide support in the duty role, including triage completion.
• Assist the Clinical Lead in effectively managing the team.
• Offering strategic recommendations to senior managers to prioritise initiatives, drive service improvements, and achieve businessobjectives and statutory requirements.

Additional Working Pattern: Must be available to work Early, Late, Night shifts and Weekends (1 in 4)
