Summary


We are looking for an experienced, confident and enthusiastic band 6 Occupational Therapist to work within the intermediate care/rehabilitation wards at Tickhill Road Hospital

The main aspect of this role is to triage patients as part of the step-up and step-down service. Assess patients and formulate individual treatment plans to ensure they meet their goals, promote safe discharges and prevent admission to the acute hospital.

We require an Occupational Therapist who is personable and has excellent communication skills. You will be expected to work independently and also inclusively with the multi-disciplinary team. You should be able to organise and prioritise your own workload and have experience of working with patients who have complex medical, memory and social issues.

We will provide support through in-house professional forums, Mandatory training, 1:1 and peer supervision. We facilitate student education and have close working relationships with neighbouring teams in both acute Health and Social Care.



You should be HCPC registered, have appropriate demonstrable post qualification experience of working with patients who have long term conditions.



It is essential that you are a car driver due to being required to completing home assessment visits.
• Provide therapeutic intervention to a group of patients within the Intermediate care case load alongside the MDT– consisting of step-up and step-down patients to promote discharge and prevent admission to the acute hospital.
• Triage referrals received from other health professionals, being aware of quality and appropriateness of referrals, and action as necessary.
• Independently manage a caseload without direct supervision and to be professionally and legally accountable for all aspects of own work, including the management of clinical risk.
• Provide supervision and developmental guidance to colleagues workingwithin the team in relation to own areas of expertise and special interest, including nursing colleagues, Rehabilitation Assistants and the band 5 Occupational Therapist
• Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.
• Participate in pathway design and promotion of the service to encourage new sources and routes of referral to optimise the scope and impact of the service.
• On occasions, assist the Occupational Therapists within other areas of the Home First service.
