Summary


An opportunity has arisen for an enthusiastic, compassionate and dynamicBand 7 District Nurseto join our District Nursing team in Milton Keynes leading a team of Registered Nurses, Nurse Associates and Health Care Support Workers.

The District Nurse will be responsible for a caseload of patients including case management of our complex and end of life patients. The District Nurse will be highly skilled, professional and able to provide holistic assessments and develop comprehensive nursing plans of care to meet the needs of each patient on caseload. They will also work in partnership with primary health care teams, social care, voluntary sectors and other CNWL services ensuring that the patient is involved and at the centre of all care provided.

Hours of service are 09:00 to 17:00, to include weekends and bank holidays on a rotational basis and being a car driver is essential.

• Teaches/coaches, demonstrates, supervises and supports nursing care to patients as required when working clinically.
• Carries out skilled evidence-based nursing intervention and ensures implementation throughout the team.
• Manages complex, challenging and demanding clinical situations.
• Ensures clinical practice of the team is up to date and based on current evidence base.
• Applies current IPC policy issues and universal precautions.
• Responsible for ensuring Community Nursing teams develop appropriate plans of care for individual patients, families/carers or patient groups.
• Leads, facilitates and disseminates information on appropriate audits to ensure outstanding care and maintenance of high standards.
• Ensures community nursing actions following MDT’s are implemented.
