Crynodeb
Teaches/coaches, demonstrates, supervises and supports nursing care to patients as required when working clinically. Carries out skilled evidence-based nursing intervention and ensures implementation throughout the team. Manages complex, challenging and demanding clinical situations. Ensures clinical practice of the team is up to date and based on current evidence base. Applies current IPC policy issues and universal precautions. Responsible for ensuring Community Nursing teams develop appropriate plans of care for individual patients, families/carers or patient groups. Leads, facilitates and disseminates information on appropriate audits to ensure outstanding care and maintenance of high standards. Ensures community nursing actions following MDTs are implemented.
