JOB DESCRIPTION JOB TITLE: Advanced Paramedic Practitioner ACCOUNTABLE TO: The Partners [Clinical Leads] RESPONSIBLE TO: Practice Management [Administration] HOURS: Full-time: 36 hours per week LOCATION: Sidley Medical Practice [Primary site] Albert Road Surgery [Branch Site] Job Summary: The role will provide a specialist resource as an advanced paramedic practitioner for our patients working with clinicians, nursing team and in collaboration with other members of the practice multidisciplinary teams. To work within our practice community, as an autonomous, accountable paramedic practitioner providing holistic care whilst assessing, diagnosing and providing treatment to patients. To use competent and up to date clinical skills. To assess, diagnose, treat, refer or direct to other services; our patients who attend surgery or request domicile visits relating to unscheduled minor conditions, injuries or acute illness. To promote self-care and educate service users to allow them to be empowered to make informed choices about treatment. The post holder must have full driving licence with suitable insurance and access to suitable vehicle. Mileage will be claimed back at the all-inclusive standard BMA Committee Rate currently set at 45 pence per mile on production of the appropriately completed expenses claim form. Job Responsibilities: Minor illness assessment, diagnosis and treatment with access to seek advice, review and onward referral within clinical teams at Sidley Medical Practice. Patients requiring care and onward referral to secondary care to be discussed with available GP and this process to be reviewed ongoing with the Partners. To see patients within clinics at the two surgeries, provide care to patients on domicile visits and to develop skills within triage and telephone consultations. To develop care plans for patients when required and to be involved with the continuity of care of our patients such as in end of life care, chronic disease management as developed and supervised with the Partners. Maintain clinical knowledge and practice to known capabilities and competencies. Ensure registration and qualifications with the Health and Care Professionals Council is maintained. Prescribe/issue medications with suitable prescribing training and supervision and within patient group directives. This is to include with development patient specific instructions such as vaccinations, nebulised medications, oxygen administration and intravenous / muscular medications in medical emergencies. To undertake wound management within competencies and resources available. Undertake the collection of pathological specimens with appropriate training and supervision including intravenous blood sampling, point of care testing and swab collection. To maintain accurate, contemporaneous and concise electronic records within the practice, having received suitable training and supervision in the use of EMIS software and read coding/template submission. Involve in ongoing education within the practice and to support and provide training to staff members as identified. Participate in annual appraisal with audit as identified and to develop and maintain Continuous Professional Development plans. To work collaboratively alongside reception and administration staff and to ensure that service user needs are met. To ensure you have adequate and appropriate indemnity in place. Administration: Participate in the administrative and professional responsibilities of the practice team. Ensure appropriate mileage claims are made accurately and submitted to the Finance Manager. Ensure all necessary paperwork and registration documentation is completed and submitted to the Quality Assurance Manager. The post holder has a legal responsibility to comply with the Health and Safety at Work Act 1974, other relevant information and any risk or safety related issues introduced by Sidley Medical Practice in the interests of staff, patients and contractors. The post holder must be aware of their responsibilities under the Health Act 2008 Code of Practice for the Prevention and control of Healthcare Associated Infections. The post holder will be shown how to access relevant polices within the practice during their induction. Sick Leave is conditional to the terms as set out in the Sidley Medical Practice Employee Handbook Issue 12 of May 2024. Annual Leave is 5 weeks pro rata of 36 hours a week being 180 hours in a complete holiday year (January December). Information relating to service users, carers, colleague other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
