Summary
Job Summary: The postholder will work as a member of the Health Care Team offering directed treatments to meet the needs of patients under indirect supervision of the Registered Practice Nurse and or General Practitioner. The following list serves to illustrate the scope and responsibilities of the post and is not intended to be exclusive. Main duties and responsibilities: 1) Clinical To provide high quality patient care To work as a supervised member of the health care team, performing delegated nursing duties, for example: Venepuncture ECGs Injections ie, Flu Urine testing Blood pressure monitoring Ordering of health promotion literature Ordering of stock from local hospital Health checks Patient recall Smoking cessation Lifestyle Advice and Education Well Person Clinic Any other duties assigned to this role Identified areas for Training will also be provided Please note this list is not exhaustive. To run health promotion and disease related clinics under the supervision of the Practice Nurse. Ensure that the care given meets the requirements of local and national guidelines, policies and standards including the Quality Outcome Framework (QoF) Respond to medical emergencies, incidents and accidents, as directed and as competent to do so. Is Trained in Basic Life Support. To undertake initial health assessments at reception and highlight concerns to Nurse or GP on duty. To be responsible for the ordering of repeat medication as prescribed by the GP, safe keeping and processes for the storing of all medicines/drugs according to NMC standards and North East Hampshire CCG. To ensure that all daily records for fridge temperature, key safe log, emergency drug box and cupboard are checked within the correct timescales and any anomalies are reported to the Medical Director in absence of the Practice Nurse. Maintain Cold Chain procedures Provide information, advice and support to patients face to face and via the telephone. Promote & participate in maintaining a safe comfortable & therapeutic environment. Involved in team discussions to ensure best practice To participate in outreach clinics providing healthcare and lifestyle advice to hard-to-reach groups To work with the ethos of North Camp Surgery with the aim of achieving the key Performance Indicators required by CCG. Use appropriate infection control procedures and maintain work areas in each clinical room so that they are clean, safe and free from hazards reporting of any potential risks identified, including: 1. hand washing 2. universal hygiene procedures 3. collection and handling of laboratory specimens 4. segregation and disposal of waste materials 5. decontamination of instruments and clinical equipment 6. reporting and treatment of sharps injuries 7. dealing with blood and body fluid spillages 8. Assist patients and colleagues in adopting sound infection control measures 2) Administration To maintain accurate records and data on Treatment Room activities in a timely fashion using EMIS clinical system. To work as an effective team member and promote a multi-agency collaborative approach to health care. Maintain the established reporting documentation and communication system to ensure efficient evaluation and monitoring. Report and record all clinical incidents to Practice Manager. Maintain and ensure the effective use of clinical stock levels 3) Communication Establish and maintain effective working relationships with individuals and other health care partnerships. To communicate effectively using a variety of communications skills: verbal, & non-verbal, written and electronic, with a range of individuals and organisations within the NHS. Make all reasonable attempts to overcome any barriers to understanding that may occur, giving due consideration to ethnicity, language, educational ability, barriers caused by illness/distress etc. which may adversely affect communication. Assess individual communication needs and understanding for informed consent. Working with interpreters [link workers] for non-English speaking patients. 4) Education, Training & Research To take responsibility for own professional development undertaking training for specific clinical tasks Participate in clinical audit Participate in research programmes as appropriate. Participate in the development and implementation of standards and audit activities. Attend all mandatory training courses and the updates and any study days as appropriate to the post Attend performance and development reviews with your line manager. General Duties: The Post Holder may be required to work additional hours to cover holidays and sickness. Maintaining regular consistent attendance, punctuality, personal appearance and adherence to relevant health and safety procedures. To attend all statutory and mandatory training courses and any courses specific to this role as discussed with the line manager. To be available for staff meetings and meetings with management. Person Specification Person Specification Essential Desirable Academic/Vocational Qualifications Educated to GCSE standard (or equivalent) with grade C or above in English Evidence of continuing professional development Good keyboard skills experience of using Microsoft office e.g. Word, Excel NVQ Level III Health & Social Care Experience of using EMIS clinical system Basic Life Support Training Experience Experience of working as a HCA in the NHS and or caring environment. Experience of working autonomously in treating patients. Experience of working in teams within a clinical team setting. Experience of working in a multidisciplinary environment with a wide range of individuals at different levels. Experience of developing relationships with external agencies and organisations. Experience of working across organisational boundaries within health or social care. At least 1 years experience of working as a HCA in the NHS and or caring environment. Experience of performing new patient health checks within general practice. Experience of working within a GP Practice. Experience of working with hard to reach group. Knowledge An understanding of the role of Primary Care Trusts within the wider NHS. An understanding of the structure for the delivery for primary health care services. An understanding of the implications of cultural difference for service delivery. An understanding of the steps that need to be taken to provide appropriate, accessible and sensitive primary health care services.
