Job Title: Personal Advisor - Leaving Care Personal Advisor Officer

Location: Wakefield Council
Salary: £16.00 per hour plus holiday pay
Contract Type: Full time - 2 month Contract

About the Role

Are you passionate about supporting young people as they transition from care to independence?

We are looking for a committed and compassionate Personal Advisor to join our Leaving Care Team. You will support young people aged 16-25 who are in or have left the care system, guiding them through education, housing, employment, and life skills to help them achieve independence.

Key Responsibilities
  • Manage a caseload of young people leaving care or post-18.

  • Lead and review Pathway Plans and Risk Assessments.

  • Offer tailored support in areas such as housing, budgeting, benefits, and personal development.

  • Work collaboratively with internal teams and external agencies to deliver a joined-up service.

  • Maintain accurate case records using IT systems.

  • Participate in meetings, duty rotas, and crisis responses.

  • Support and promote the voice and participation of young people.
