Are you passionate about supporting young people as they transition from care to independence?
We are looking for a committed and compassionate Personal Advisor to join our Leaving Care Team. You will support young people aged 16-25 who are in or have left the care system, guiding them through education, housing, employment, and life skills to help them achieve independence.
Key Responsibilities-
Manage a caseload of young people leaving care or post-18.

Lead and review Pathway Plans and Risk Assessments.

Offer tailored support in areas such as housing, budgeting, benefits, and personal development.

Work collaboratively with internal teams and external agencies to deliver a joined-up service.

Maintain accurate case records using IT systems.

Participate in meetings, duty rotas, and crisis responses.

Support and promote the voice and participation of young people.
