Crynodeb
The right candidate should be forward thinking, able to work on their own as well as part of the team as the role will involve being responsible for risk assessment and management, care planning and care delivery within a team approach. The post holder should be confident in assessing and managing mental health and social needs and be able to deliver a range of therapeutic techniques. Devising care plans in partnership with the people that are referred, and their carers/families is essential. The post holder will work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met
