To participate as a team member in the setting and reviewing of therapeutic aims, objectives, standards and strategies to provide a high standard of care to clients and their families. To work at a variety of levels, including individual and group work and have a flexible and innovative approach to prevention and early intervention work. To link with other statutory and voluntary agencies and professionals, as well as CAMHS teams, in terms of referrals and joint work. Maintain good relationships and clear lines of communication with other professional agencies and departments, providing specialist advice and support where appropriate. To ensure that all recording and reporting of client contact is accurate, up to date and of good quality and within the Trust and professional requirement standards of record keeping. To collect and collate information about individual clinical activity which will be submitted to team manager. To have IT and data entry skills to enter clinical information into databases. Work in accordance with and within the principles/guidelines of all aspects of clinical governance. Identify development and training needs with manager in accordance with Trust guidelines. Remain up to date with changing practices and wherever possible make full use of all training and educational facilities. Participate in the provision of a good quality learning environment for trainee staff, overseeing placements and offering supervision to trainees. To attend supervision and appraisal meetings. To make effective use of colleagues in sharing and participating in casework supervision and other forms of supervision when required according to local practice, including attending peer supervision meeting. To maintain and ensure good professional standards and attitudes towards the care and treatment of adults and their families. To participate in the monitoring, evaluation and audit of the service. To work with others to undertake and promote research-based practice. To join colleagues in specialist teaching, consultation/support to other professionals and agencies. To undertake all duties in line with the Codes of Professional Conduct, professional policies and guidelines and Trust governance standards. To maintain professional registration requirements for the post according to discipline/profession. Clinical Individual Assume responsibility for own case list and case management of adults and families. Providing assessment and treatment according to current standards of practice and consistent with agreed team approaches to case management. To regularly visit parents/carers and their families at home or in other community settings or statutory agency areas as required. To participate in and lead joint complex assessments with other disciplines and other agencies and provide reports for referrers, families and other agencies as required. To make a formulation from an assessment, including risk assessment, to guide intervention. Create, and share copies of the care plan as clinically appropriate. To assess any child safeguarding concerns for own case and consult or refer to Childrens Social Care/Family Early Help. Seek and share advice and support regarding safeguarding issues within the team. Maintain good relationships and clear lines of communication with other professional agencies and departments, providing specialist advice and support where appropriate. To facilitate or co-facilitate groups such as the Keeping Well Post- Birth group and parent/child wellbeing group. Management responsibilities Contribute to the effective functioning of the team and service through attendance at relevant meetings, taking responsibilities within the service as appropriate and negotiated with the line manager. Participate in the provision of a good quality learning environment for students and staff within team. To be responsible for the management of financial and physical resources for yourself and to have an awareness of resources used within the team. To ensure all relevant data is collected and provided to the commissioners as requested.
