Summary
Job Purpose 9-5 Monday-Friday
The post holder will work autonomously and in collaboration with the community teams within CLCH, other health care professionals, statutory and voluntary agencies to deliver a high-quality physiotherapy service to children and young people in the Merton Community.
· With a variety of conditions including complex needs, profound and multiple learning disabilities, and acute rehabilitation.
· In the community setting which is most appropriate for that child i.e., clinic, school, nursery, patient’s home, children centres or special school.
· To perform an advanced assessment of patients including those with highly complex presentations and from this formulate a problem lists and treatment plan, which is carried out as an autonomous practitioner.
· To carry a clinical caseload and work independently as a clinical lead.
· To take a lead role in the supervision, line management, appraisal and teaching of junior physiotherapists, students and MDT staff in physiotherapeutic care of children with a variety of conditions.
· To develop and promote a culture of continuous improvement and innovative practice with the physiotherapy team supervise, initiate, and participate in evidence-based projects and be responsible for implementing changes within the team/area and setting and monitoring standards of practice.
· To assist the Team Leader on data analysis and reporting for the physiotherapy team.
· To be a clinical lead for the community physiotherapy service, providing supervision, and support to the team
· To assist the Team Leaders and Professional Lead for Physiotherapy in planning and providing appropriate physiotherapy to children in the community setting including attendance at appropriate meetings, developing guidelines and protocols and reviewing service provision.
· To keep the Team Leader up-to date with service provision issues including clinical activity, service pressures versus service capacity and DNA rates; and report any untoward incidences clinical or non-clinical, promptly, cooperating with any investigation undertaken.
· Contribute to reports on Paediatric Physiotherapy Service in Merton. These are to be completed in a timely manner and to a high standard.
· To actively review efficacy of service delivery and facilitate setting up of new standards or amend existing ones in close liaison with the Team.
· To keep up to date with the department and Trust standards and policies and facilitate dissemination of this knowledge to colleagues.
· To develop good working relationships within the setting, multidisciplinary teams, outside agencies and voluntary agencies to ensure that professionalism and service delivery are maintained.
