Crynodeb
The main role of the successful applicant will be to proactively communicate and coordinate information between patients, their carers, doctors and professionals and to assist the clinical teams with administrative functions within the practice. Post holder will holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person.
