Summary

We are looking for an experienced Adult's Social Worker to join a Joint Emergency Team.

It is required to have a Social Work Qualification with at least 2 years of post qualified experience.

About the Team

The team is responsible for responding to emergency enquiries out of working hours. A significant aim of the team is to ensure people are supported within their own homes and to avoid the need to go to hospital. Recently, the team has been responsible for completing Continuing Health Care assessments for those that do not have an allocated social care member of staff. The team consists of expert practitioners in Nursing, Occupational Therapy, Physiotherapists, General Practitioners and Social Workers.

About You

Experience working within an Emergency Duty Team and CHC assessment experience is essential for this role. It is necessary to have a degree within Social Work (Degree/ DipSW/CQSW) with a minimum of 2 years post qualified experience. a valid UK driving licence is preferred but not essential.
