Summary

The Health and Wellbeing Coach will be a key part of the practices multidisciplinary team, working closely with patients who have or are at risk of developing metabolic conditions. You will deliver tailored interventions that address the root causes of metabolic health issues, including poor nutrition, lack of physical activity, stress, and unhealthy lifestyle habits. This role is perfect for individuals passionate about empowering patients to take control of their health and improve their quality of life. Key Responsibilities Personalised Coaching: Provide one-on-one coaching to patients focusing on improving their metabolic health. This includes helping patients develop and achieve goals related to nutrition, exercise, stress management, and other lifestyle factors. Motivational Interviewing: Use evidence-based techniques such as motivational interviewing to help patients recognize and overcome barriers to behavioural change, fostering a collaborative approach to managing their health. Collaboration: To deliver comprehensive and seamless patient care, collaborating closely with colleagues in the Metabolic Health Clinic and throughout Westrop Medical Practice, ensuring a holistic and integrated approach. Metabolic Health Education: Educate patients on key aspects of metabolic health, including understanding the role of diet, exercise, and sleep in managing metabolic conditions like diabetes and obesity. Outcome Monitoring: Track and report on patient progress through regular check-ins and follow-up sessions, adjusting interventions as needed to ensure the best possible outcomes. Support Groups: Lead or co-facilitate group sessions for patients, creating a supportive environment for sharing experiences and strategies for managing metabolic conditions. Patient Advocacy: Act as an advocate for patients within the clinic, ensuring they feel supported throughout their health journey and encouraging active participation in their care plans. Desirable Attributes Metabolic Health Knowledge: Understanding of the impact of lifestyle factors on metabolic health and how they contribute to conditions like diabetes, hypertension, and obesity. Nutrition Knowledge: Basic understanding of nutrition and its role in managing and preventing metabolic diseases. Holistic Care Experience: Experience in integrating lifestyle interventions with clinical treatments in a multi-disciplinary team setting.
