To contribute to the delivery of care on shift including end of life care if required. Mentor new staff in relation to LTV care. The post holder will work in collaboration with other services providing care and support for the child / young person and family. To work as part of a team supporting a ventilator dependent child within the hospice/ community setting and to continue that support independently within the hospice and wider community. To be a designated lead for LTV care. Contribute to LTV service and care developments, including policy and procedures, under the clinical direction of the locality Matrons to ensure provision of holistic care and support to children with LTV care needs. Deliver LTV teaching, training and assessment of competency within EACH and also when required to in the community via commissioned or charitable agreement. Develop and deliver teaching, training and assessment of competency in care team staff through working alongside them in a range of settings in the community and at the hospices. Work collaboratively with other services providing LTV care packages for the child and family as necessary to ensure continuity of delivery of consistent and safe care across agencies. To deliver care on shift as required and ensure that holistic care is family centred, needs led, evidence-based and meets all statutory requirements. To apply clinical decision making and practice at a level that reflects skill and competencies in relation to LTV practice. Seek opportunity to be an active member of a quality and safety group. To assess, plan, implement and evaluate RAHAs and care plans to meet the diverse and individual needs of children and families. To contribute to the management of BCYP caseload with LTV requirements in conjunction with the care team. To make an active contribution to the on-going development of EACH care services in relation to LTV practice. To work proficiently and effectively within the relevant legal, ethical and professional frameworks applicable to childrens palliative care practice in the community and in the hospice at all times, including the NMC Code of Professional Conduct. To be responsible for maintaining and updating own professional practice by actively participating in reflective practice, appraisal, management and practice supervision and to be a full and willing participant in opportunities for learning. To act as an ambassador for the organisation and to maintain positive and effective communication with other agencies involved with families receiving EACH services. To attend discharge planning, review and debrief meetings as appropriate. To undertake any other duties as are within the scope, spirit and purpose of the job, the title of the post and its grading, as requested by your line manager or someone acting on their behalf.
