Crynodeb
You will be responsible for achieving positive change and improved outcomes by supporting the team. Plus we work closely with our PCN teams to ensure integration of services and support. You will also contribute to the development of knowledge and promotion of excellence in social care using evidence-informed practice. You will make use of sophisticated, critical reasoning and both model and facilitate reflective and evidence informed practice, applying a range of tools to provide innovative and appropriate solutions to support the team.
