Crynodeb
The Practice nurse will be part of the nursing team responsible for ensuring that the practice provides an optimum service of nursing care relevant to the practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative health care alongside the day to day duties required, providing a general nursing service to the practice population. The role requires initiative and good judgement and will be supported by other clinicians such as GPs and Nurse Practitioners. The role also requires the practice nurse to take a strategic role within primary care team creating links with other community health providers as required and to provide general assistance to the Practice team and project a positive and friendly image to patients and other visitors. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required
