Summary
· Promote effective patient flow based on individual service users care pathways whilst ensuring least restrictive practice and optimizing lengths of stay and support the PCFT Trust wide goal to reduce out of area placements.
· To promote and facilitate timely admissions and discharges within inpatient unit through safe, timely and effective bed management as an integral part of the CPA process. To operate as a member of Inpatient Multi-disciplinary team to provide clinical expertise to facilitate safe discharge from the inpatient service.
· To liaise closely and effectively with key staff in other agencies HBTT, CMHT, EIT, Older people’s services, LD and Rehabilitation Services etc to maximize safe and timely discharges
· To act as a resource to provide education, advice and support relating to all aspects of admissions and discharge planning and provide supervision and training as required to junior members and students of the PCFT Trust Wide Patient Flow Service.
· Participate in relevant PCFT Trust Wide Patient Flow meetings and provide cover as required for the Hub.
· The hub team will work shifts to provide cover 7 days per week.
· The post holder will chair the local bed management meetings and board rounds and keep accurate records.
· Provide clinical expertise regarding admissionanddischargecriteria andensure bed management policies and proceduresare adhered to.
· Gatekeep allreferralstotheinpatientservicesensuring theleastrestrictiveoptionsare applied at all times to manage service users, complete required documentation and update service users’ electronic clinical records as required.
· Workwithreferrerstoascertain anyalternativetotherequestedadmission.
· Workinconjunctionwithexistingcarecoordinatorstofacilitatedischarge.
· Developlinkswithhousingproviderservicesensuringthatproceduresare implemented: for example homelessness.
· AttendCPA,MDTandWardRoundMeetingsasrequired.
· Ensure inpatient CPA pathway is implemented for all service users, inpatient CPA care levels are reviewed and updated as appropriate and discharge CPA’s including crisis plans are completed at point of discharged and communicated with key individuals in the absence of a care coordinator.
· EnsureKPI’s,andrelevantCQUINandorCQCtargetsarecompletedandmaintained.
· Undertakeclinicalassessmentsasnecessary,includingpatientsplacedinother hospitals (NHS and private providers).
· Undertake assessmentsonserviceusers inhospital whomaybenefitfromearly supported discharge and to explore this option with the MDT, service user and their carer where appropriate.

· ProvideinformationtosupportresponsestotheICBandothersasisrequired.
· Be able to communicate effectivelyand fluentlyto otherprofessionals and signpost where necessary.
