Delivering time-limited recovery programmes using digital platforms. Accepting referrals, assessing need, and developing recovery-focused care plans. Providing health and harm reduction advice, including information about alcohol use and blood borne viruses (BBVs). Creating and facilitating remote content and digital intervention materials. Referring to Inclusion hubs or other services for in-person or ongoing support as needed. Maintaining accurate and timely digital records of all activity. Working closely with colleagues and external partners to support person-centred care and reduce harm. This role combines frontline recovery work with service innovation, helping shape how digital alcohol support is delivered across Hampshire.
