Your responsibilities Clinical Practice and Quality Practice in compliance with your respective code of professional conduct and scope of practice, being responsible and accountable for your decisions, actions, and omissions at this level of practice Demonstrate a critical understanding of your level of responsibility and autonomy, and the limits of your own competence and scope of practice, including when working with complexity, risk, uncertainty, and incomplete information Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change Actively contribute to implementing the quality governance systems and processes across the organisation, demonstrating receptiveness to challenge and learn, and preparedness to constructively challenge others, in doing so. This includes but is not limited to healthcare records, learning points, and significant events Adopt a solutions and improvement mindset to proactively assess the effectiveness of your care delivery through self and peer review, benchmarking and formal evaluation. Clinical Assessment and Patient Care Participate in and undertake triage of urgent care requests to ensure the smooth progress of patients through the system on a daily basis. Work in partnership with patients, families and carers, including care homes, using a range of assessment methods, your expertise, and your clinical reasoning to adopt an evidence-based approach compliant with NICE guidelines to: - Assess, examine, diagnose, plan, implement and evaluate holistic treatment, interventions and care for patients presenting with an undifferentiated diagnosis -Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of emergency care - Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate - Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan Demonstrate effective communication skills to optimise patient satisfaction while managing their expectations and supporting them to make decisions, plan care, or make positive change, including: To work with patients to ensure that individual medication regimes meet their needs and are aligned with relevant guidelines and regulations for prescribed treatments. To ensure appropriate use of prescribed and over-the-counter medication, and understanding of medication regimens, side-effects and interactions When dealing with sensitive matters that may also have legal or ethical considerations, for example, safeguarding, substance abuse, sexual orientation and gender identity, racial and ethnic diversity, mental illness, disabilities, etc To support patients to adopt health promotion strategies, and apply principles of self-care, including referring or sign-posting to other services that may support their wider well-being, e.g. Social Prescribers, Health & Well-being coaches, etc. Work collaboratively with your colleagues within Mereside practices and across the local health economy to manage risks and issues, and integrate effectively with health and social care providers, ensuring consistency of care across settings and practices. Learning, Development, and Education Critically assess, and take responsibility for, your own learning and professional development plan that reflects the breadth of the needs of advanced clinical practice in primary care Proactively share learning, information, and good practice insights across the practice and wider organisation to facilitate individual and team learning Advocate for, and positively contribute to, a cohesive culture of organisational learning and collaboration both within and between teams, to ensure the best patient experience throughout their entire journey. Leadership and teamwork Contribute positively to a sense of cohesion, and foster a culture of wider collaboration, across the organisation, engaging effectively with collaboration tools such as MS Teams to improve communication and information sharing across sites and teams, including remote workers Work as an effective, responsible, and supportive member of the Duty Team, collaborating well with diverse colleagues and constantly seeking to improve ways of working both within the team and throughout the patient journey to ensure efficient and effective delivery of care Offer mentorship and support to junior colleagues as per the Mereside AP framework. Prioritise own workload and ensure effective time management strategies are embedded within the culture of the team to ensure that cases are appropriately triaged, handled by the most clinically appropriate member of the team, and completed in a timely manner.
