The post is an ideal opportunity for a newly qualified Allied Health Professional or an experienced Band 5 who is looking to gain further experience in their clinical skills. Working within the Older People’s Mental Health Service line and reporting to the Senior Practitioner (B7 OT) SOAMHS – Borough based.
The focus of the post is to carry out comprehensive assessments and provide evidence-based interventions to patients and carers, and to supervise and clinically support more junior members of the multidisciplinary team including students.
The interventions will include education, individual work and group work and will involve working closely with patients, carers, statutory and voluntary agencies, within a multi-disciplinary team. They will hold their own complex caseload of patients.
The post holder will be based in one of the community teams in Hounslow and they will be undertaking home visits, meetings at the team base and other community sites.
