Summary
The role of ANP requires an experienced nurse who, acting within her / his own professional boundary, will provide care for patients within all the Nursing and Care homes in the localities, and all other patients, within their own homes who have been referred into the scheme by GP practices within the above boundaries. Acting as an autonomous practitioner working independently, and in conjunction with, other health care professionals to assess, investigate, diagnose, treat and evaluate all patients seen, including referring into other services wherever or whenever required. The post holder will demonstrate safe, clinical decision-making and will provide expert professional advice to patients, carers and colleagues ensuring maintenance of clinical excellence. The ANP will work collaboratively with the general practice team to meet the needs of patients. They will provide education and training to other staff and students, and provide leadership as required.
