As our Lead Support Worker, you’ll be at the heart of supporting our customers who have complex needs to support them within their accommodations and out in the community. Our Central Resource Team supports our supported accommodations to cover staff annual leave, sickness or recruitment to ensure our customers always get the best possible service and support.

Typical day as a Lead Support Worker (known by us as Coordinator)

  • Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
  • Assisting our customers with complex needs and vulnerabilities. From homelessness, substance misuse, complex mental health and social isolation.
  • Helping customers with independent living, daily tasks, budgeting and finance, health and wellbeing and activities.
  • Carrying out risk assessments, support planning, goal setting, safeguarding and regular reviews. We also assist with ending and setting up tenancies for our accommodations.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working towards the plans.
  • Experience of coordinating and assessing customer referrals.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
  • To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Our team

You’ll join Yvonne on the central resource team who’s worked for Home Group since 2014. Yvonne started as a Support Worker and took full advantage of all the training and experience we offer here at Home Group. Supporting her is our Client Service Manager for Yorkshire, Laura who started out as a Support Coordinator within the Central Resource Team and is always available to support and answer any questions.

Job details

  • 37.5 hours a week (but we will consider part time applicants). You will work on a rota based system including evenings and weekends.
  • Travel and work between our services across West Yorkshire including Huddersfield, Wakefield and Leeds.
  • Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • Although you may lone work at times, we will get together regularly for training and team meetings.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.
