Crynodeb

The post holder will provide advanced and expert level of practice (including non-medical prescribing)within the Home Based Treatment Team (HTT) leading to improved patient outcomes. For Trainee Advanced Nurse Practitioners, it is essential that you reach the criteria in the general Apprenticeship eligibility. The Apprentice ACP will act as a professional resource for the multi-disciplinaryteam, patients and carers within the scope of their practice and competence, The Apprentice ACP willunderstand professional accountability and legal frameworks for advanced practice and be professionallyaccountable and legally responsible for all aspects of own work. The Apprentice ACP will demonstrate safepractice in line with Trust policies and procedures. The post holder will support the clinical and operational team to deliver on service improvements in thequality of care and safety through service development, evaluation, and audit. They will be an integral part of the MDT supporting clinical decision making and supporting the servicepathways, developing nurse led clinics and providing brief interventions. The Advanced Practitioner will hold a case load and will lead assessment, formulation and prescribing ofcare planning including discharge planning within agreed protocols and procedures. The post holder may additionally lead on practice and service developments. The post holder is required todemonstrate clinical and practice expertise via interventions, supervision and modelling good practice. To support improving workforce knowledge and practice through the contribution to a HTT education andtraining programme across Urgent Care teams. The core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may berequired. Advanced Clinical Practitioner (ACP) positionsshould be accessible to bothnurses and Allied Health Professionals (AHPs) in line with the Multi-Professional framework.
