Summary


The Macmillan 1-1 Primary Care Nurse will provide support to maximise the quality of life for people living with and beyond cancer through the development and delivery of community-based, personalised care and support across the borough of Oldham.

The post holder will encourage patients to self-manage their condition effectively.

The post holder will recognise the possible signs of deterioration, recurrence, or long-term effects of treatment and will have the ability to access generalist and specialist services as needed.

The post holder will complete Holistic Needs Assessments (HNA) with patients, agree on an action-based care plan, and signpost to appropriate services for additional support. This will form part of the Cancer Care Review (CCR), which will be rolled out for all cancer patients within the first three months of diagnosis and annually thereafter, for those registered with an Oldham GP.

The post holder will work as an integral member of the wider community multidisciplinary team (MDT), supporting the Specialist Palliative Care Team (SPCT) and District Nurse Team (DNT) with stable palliative cancer patients. This includes stratified risk assessment and monitoring for those at risk of pressure damage, as well as identifying signs of deterioration for timely referral into the SPCT and DNT for specialist palliative intervention, end-of-life care planning, and delivery.

The post holder will support the Team Leader in the day-to-day leadership of the team.

About the Role

As a key member of our primary care team, you will:
• Complete Macmillan Holistic Needs Assessments (HNA) for new diagnoses and ad-hoc referrals
• Collaborate with patients to create personalised care plans, identifying current and future needs
• Deliver coaching to promote active, healthy lifestyles, using motivational interviewing skills
• Educate patients and carers on recognising signs and symptoms needing concern, including long-term effects
• Review and update care plans regularly
• Share HNA summaries with GPs and advise on coding as a Cancer Care Review
• Oversee ongoing review, advanced care planning, and risk assessments for patients with palliative diagnoses who are stable
• Stay informed about NHS Long Term Plan for cancer and its focus on delivering personalised care in primary care, moving monitoring from secondary care
