The post holder will be a Registered Healthcare Professional, be an independent Non-Medical Prescriber and have extensive experience in practicing both within a team and autonomously. The post holder will be responsible for incorporating physical assessment, diagnostic testing and interpretation and formulation of treatment plans whilst avoiding hospital admission. The 3 aspects of Acute Care at Home are CRIS 2hr Urgent Community Response, Virtual Wards and Acute Visiting Service. This post will be expected to support all 3 aspects but be predominantly available to respond to AVS referrals from Primary Care, to support patients within their own homes who are unable to attend a GP appointment or need same day intervention to reduce the risk of hospital attendance. The post holder will initiate, manage and drive change in practice, for the benefit of patient care in line with current trust and local programmes and in association with the NHS Long Term plan. This is in in particular reference to supporting patients care closer to home as part of admission avoidance and Acute Care at Home. The aim of the Acute Visiting Service is to: Ensure that unnecessary A&E attendances and subsequent Admissions are avoided by delivering prompt care within the patients home for a specific cohort, that are able to receive care within their own home safely. Work in collaboration with system partners, particularly the urgent care pathway providers, including Acute Care at Home,111, WMAS. To support GP practice resilience in times of capacity surge, where an urgent visit is requested / required. Ensure there is equity of access for patients across all GP practices at the same time as ensuring capacity is utilised at all times. The Provider shall share usage reports quarterly with the commissioner and practices. Ensure the needs of the patients are met in the most clinically appropriate way through fast and effective triage. Improve patient experience and outcomes by not exposing individuals to long waits in a hospital setting and reducing the risk of hospital acquired infections. The overarching aim is to deliver a streamlined service so that patients are seen by the right professional, at the right time, in the right place. For all leadership roles (Consultants/Clinical Leads and AfC Band 7 and above) Support and promote the development of Improving Together capability within your team and its application in delivery of service outputs. Utilise the Improving Together tools, routines and behaviours to coach and develop the capability of your team to do a little every day to improve and deliver exceptional care aligned to the UHNM strategic priority domains. KEY RESPONSIBILITIES The Acute Care at Home Service provides care to patients in their own home, who historically would have required hospital treatment. The Band 7 Clinical Practitioner will work autonomously to assist with the day to day running of the Acute Visiting Service, whilst also providing acute assessment, interventions and formulation of medical management plans for individual patients. A list of main tasks and duties are outlined below but are not exhaustive due to the evolving nature of this unique role and service. Recognise and work within own competence, and to the standards of the health cares professionals code of conduct. Establishes and maintains effective working relationships, both internal and external in order to meet the requirements of the service. Demonstrates clinical expertise and leadership qualities on a daily basis in order to support the overall running of the service, supporting the Clinical Lead ACPs & Virtual Ward Matrons provide leadership to the clinical team. Confidently leads on Ward Rounds, Daily Huddles and Multidisciplinary Team meetings, establishing and maintaining effective working relationships. In order to formulate appropriate management plans for patients, whilst continuously revaluating initial findings and clinical response. Is able to confidently triage new referrals and establish the level of support the individual may require from the service. Is able to undertake a physical examination assessment at an advanced level, as indicated by the patients condition. Including formulating, implementing and evaluating treatment plans for individuals, families and carers. Prescribing and deprescribing of medications for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence-based practice and national and local protocols and within the roles scope of practice and legal framework. Is able to take the lead in complex case management, providing support and advice to others. Develops and maintains skills and knowledge to deliver a range of clinical procedures appropriate to the history and physical examination, including (but not limited to) catheterisation, venepuncture and cannulation and ECG within a patients own home. Setting an example of good practice to the wider team. Communicate effectively with patients, carers, colleagues and others using appropriate communication styles. Anticipate barriers to communication and ensure patients and significant others are kept fully informed and consent to treatment. Maintain sensitivity, offering advice, reassurance and empathy, in particular when imparting potentially distressing information, or managing uncooperative, or challenging behaviours. Supporting the 2hr Urgent Care Response with initial visits, physical health assessments and prescribing to ensure where possible admission avoidance Working with the Senior Liaison Practitioner (UHNM) to ensure appropriate patients are pulled and stepped down from ED, Emergency Portals, General Medicine Wards, and Specialist Wards Producing accurate and complete documentation and patient records consistent with legislation, policies and procedures. Supports in the planning and organisation of staff delegation, ensuring activity for the patients is delivered by staff with relevant education, expertise and competence. Provides advice and education to the individuals being cared for within the 2hr UCR or VW, families and care staff regarding treatment and management of the individual. Contributes to the induction of new staff, training of pre-registration students and participates in staff development including delivering training and education sessions, ranging from one to one, or small groups. Identifies own training/educational needs as part of the appraisal process, maintaining an agreed level of expertise. Whilst also supporting the Clinical Lead ACPs to ensure all ACAH staff take part in the yearly appraisal process. Participates and leads the team in undertaking clinical audit programmes for the ward, in order to monitor effectiveness. Working in partnership with the senior leadership team, drives service development and proposes changes to practice. Including contributing to implementation and promotion of policies, procedures and guidelines. Possess excellent time management and personal organisation skills. To take responsibility for the safe use and transportation of equipment between locations. To work in non-purpose-built rooms involving the necessity to move furniture, therapy equipment etc. Ensure appropriate representation and participation in departmental meetings as appropriate for role.
