Summary
Conduct a care plan review, using the review template provided by the ICB to ensure consistency and standardisation across all care homes. The review shall involve the patients carers and or family if available and appropriate and include, but not limited to the following: Basic Observations. Information gathering. Must score 2 or above refer to the dietician PHQ9 if required Six item CIT if required Where appropriate the patients preferences must be documented for end of life, care and resuscitation status of the patient established and documented. Where appropriate discuss the findings from the comprehensive assessment with the Care Home to ensure the important aspects of shared care are co-ordinated for the benefit of improved patient care. These discussions should be documented in the Patient's medical records. Ensure all residents have a Personalised Care and Support Plan (PCSP) Ensure all residents have an Advanced Care Plan Death Audit Support Team in administering Influenza and COVID vaccinations. Working within and supporting a multidisciplinary team (MDT) approach Attend weekly MDT meeting on a Tuesday Admin support for MDT Meetings
