Job Purpose:
To plan, deliver and support engaging, meaningful activities for individuals living with dementia, promoting wellbeing, inclusion, and independence. This role helps reduce isolation and supports people to maintain cognitive function, physical activity, and social interaction in a dementia-friendly environment.
Key Responsibilities:
Plan and deliver a range of dementia-friendly activities (e.g. arts and crafts, music, reminiscence sessions, gentle exercise).
Support the coordination of weekly group sessions including Merry Moments and Social Circle.
Work one-to-one with individuals to provide tailored activity support where needed.
Create a welcoming, safe and supportive environment for all participants.
Build positive relationships with individuals, families, and carers.
Record attendance, collect feedback, and contribute to monitoring outcomes and progress.
Assist with setting up and clearing activity spaces.
Promote the service and encourage referrals from the community, health professionals and partner organisations.
Attend training, team meetings, and supervisions as required.
Person Specification:
Essential:
Experience working with people living with dementia or in a similar support role.
Friendly, compassionate and patient approach.
Good communication and interpersonal skills.
Ability to plan and facilitate group activities.
A flexible, proactive attitude and a team player.
Understanding of the needs of older people and those living with dementia.
Willingness to undertake relevant training (e.g. Dementia Friends, safeguarding).
Desirable:
Relevant health, care or community qualification.
Experience in arts, music, sports or wellbeing-based activities.
Access to own transport for outreach sessions.
