Crynodeb
The post holder will provide clinical support and assessment working in partnership with health and social care teams across a wide range of settings, including domiciliary/therapy led care in the community. The team provide an in reach into our acute hospital to achieve a timely discharge into our step down beds or care home provision, with a Home First Ethos as a trusted assessor
