The Lead PBS Nurse will be a member of the intensive support team and responsible for the delivery of safe and effective nursing practise within the team, this includes the allocation of the intensive support team to meet local demands and to support individuals to remain in/ return to their communities where ever possible. The Lead PBS Nurse will ensure clinical systems are in place to support and monitor practice and performance and support the team manager in the operational delivery of the service. The Lead PBS Nurse will hold their own clinical caseload and deliver highly specialist clinical care. Working with other intensive support lead Nurses, the post holder will provide trust wide leadership and support within their team. The intensive support nurse will be an expert in positive behavioural approaches and provide support and advice to the wider teams as required. KEY RESULT AREAS: Clinical: The post holder will carry a caseload providing highly specialist nursing support to individuals who require intensive support. To ensure nursing interventions are of a high professional standard, informed by evidence based practice and involves the service user and carer in all aspects of the care programme. To undertake specific standard and non standardised assessments to formulate the overall treatment plan in conjunction with service users and carers. To be responsible for the allocation of work within the nursing Intensive support workforce including the deployment of staff across 24/7 rota where necessary to support an individuals needs. To assess and manage risk, identifying needs and issues to the broader MDT. To undertake specialist behavioural assessments and lead in the development and implementation of positive behaviour support plans. To act as care co-ordinator where appropriate. To support a co- ordinated team approach throughout the assessment and in meeting the service users needs including developing and maintaining close working relationships with all services and agencies. To regularly evaluate the effectiveness of clinical interventions by undertaking specific audits using agreed outcome measures. To utilise clinical interventions that work towards and support people to live independently within the community. To work effectively within the multidisciplinary team ensuring that both written and verbal communication is clearly maintained. To be responsible for the maintenance of appropriate clinical records both written and electronic. To arrange/ attend appropriate clinical meetings. To provide accurate and timely clinical reports and data as required. To participate in clinical supervision . Locality Leadership To be responsible for the effective allocation of the intensive support nursing workforce to meet the support needs of service users, identifying pressures to the Intensive support team lead. To ensure all significant events relevant clinical issues and difficulties that arise are communicated to the Operational Team Manager /collective leadership team To direct work to the other nursing team members giving due consideration to their specific skills and abilities. To ensure an effective system for appraisal and supervision is in place within the nursing workforce. To participate in the recruitment and retention of staff. To contribute to the development of the locality and service line annual plan and other improvement processes. Trust wide Leadership Alongside other senior nurses provide trust wideleadership and advice specific to the nursing profession. To act as a source of professional expertise to other locality teams where required. To maintain a high level of skill and expertise in positive behavioural approaches and share this with others through both formal and informal structures. To lead in staff and student training and development in relation to specific nursing expertise and contribute to training of the multi disciplinary team. To advise the leadership Trio of relevant national policy and guidance specific to the postholders profession/ special interests To participate in the investigation of complaints and other formal processes as directed by the Operational Team Manager/General Manager.
