Crynodeb
To work autonomously in the provision of high-quality specialist, evidence-based care for people with diabetes, from diagnosis to long-term management To promote self-care management to patients to prevent worsening of their condition and improve health outcomes. To ensure full understanding of QOF for diabetes chronic disease management and to liaise with the team to ensure targets are achieved. To actively collect and maintain data that will demonstrate patient improvement in key areas. To ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate Primary Care Network and local population objectives. To act as a specialist resource and provide specialist advice in relation to diabetes, to patients their family/carers and health professionals. To work with the existing diabetes specialist nurse on the various workstreams created for the PCN. To work with the PCN to monitor progress and performance against three treatment targets. To support the upskilling of the practice staff to raise the standards of general diabetes care and awareness across the patch. Engage in different initiatives to engage patients including patient education sessions and group consultations.
