Crynodeb
To coordinate, facilitate and carry out individual and group activity programmes with service users. Care will be planned under the supervision of senior members of the Team. Creating positive working relationships with local services and local activity providers covered by the Trust, as well as creating and compiling resources within the service. To provide support to families and carers, including assisting other colleagues in the safe running of the service. To provide support for service users around eating and meal preparation in their own home and within the local community To attend and contribute to clinical team meetings and handovers across the Liaison and emergency department services. You will support senior team members with welcoming, conducting baseline observations following referrals.
