Crynodeb
This a new and exciting team of Nurses and Physiotherapists and Occupational therapists. The team includes doctors, specialist nurses and therapists who care for frail and elderly patients in the Emergency Department, the Acute Admissions Unit and throughout the hospital. The development of virtual frailty wards aims to allow the team to support patients in their own homes via early supported discharge or admission avoidance processes.The Frailty Virtual Ward of Newham Hospital provide safe care in the patient’s own home to avoid admission into the acute hospitals or facilitate safe, earlier timely discharges for those patients where ongoing management can be provided with clinical and care support in their place of residence. The unit aims improve patient experience and outcomes.
The Senior Physiotherapist is a member of Newham University Hospital’s virtual frailty ward team delivering multidisciplinary assessment and clinicalmanagement for frail patients within the hospital, the emergency department and in the community.The post holderAssesses, plans, implements and evaluates care for patients with frailty needs and make changes as necessary.

Although this position is a 10 months fixed-term contract, the actual contract duration will be adjusted to account for the time taken to advertise the post, complete the hiring process, and finalise pre-employment checks. This means the contract will begin when the candidate starts the role and may be slightly shorter than 10 months.
