Would you like to work in an incredibly varied, challenging, and rewarding role? If so, we have a fantastic opportunity for you to join Suffolk County Council's West Sudbury Team as an Independence and Wellbeing Practitioner, working from the Sudbury Locality Area.

Your role
We are seeking a dedicated practitioner with a positive mindset to join our team in the West of Suffolk. In this role, you will work alongside colleagues both within and outside the organisation, including health colleagues, allocations lead, Business, Finance and Service Development and Contracts teams, and wider partners. This position offers an ideal opportunity to develop and enhance your experience in Adult Social Care.
The main purpose of this role is to work with adults to identify and deliver high-quality outcomes that support and maximise independence and wellbeing.

Key responsibilities
- Work as part of a team to promote personalisation, independence, wellbeing, and choice for adults.
- Conduct asset-based assessments and reviews.
- Create reablement plans in line with a signs of safety solution-focused approach.
