To work in partnership with multi-disciplinary colleagues in health, social care and education to ensure all holistic needs of the CYP are being met. To work in partnership with the CYP and their family delivering holistic care with the support of a named nurse. Participate in the review of individual healthcare plans in association with the named nurse, CYP and family. To work flexibly over a 24-hour period, 365 days a year, including to work nights/ weekends/ bank holiday shifts. The post holder must meet travel requirements of the post. To work within a wide range of locations and across the County of Warwickshire. The post holder will be required to have access to independent means of transport for work purposes to travel across Warwickshire county and to other agencies. Promoting Health and Preventing Ill Health Support CYP to improve and maintain their mental, physical, behavioural health and wellbeing where possible dependent on the trajectory of the CYPs condition. Active involvement in the prevention of and protection against disease and ill health Engage in public health, community development, and in the reduction of health inequalities Support health and social care colleagues with completing assessment of CYP. Provide and Monitor Care Provide compassionate, safe and effective care and support to CYP in a range of care settings. Undertake training and complete competencies for specific nursing skills in order to facilitate care provision and assessment. Monitor the condition and health needs of CYP within their care on a continual basis in partnership with people, families, and carers Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care Recognise and report any situations, behaviours or errors that could result in poor care outcomes The post holder will be required to use a computer, either stand alone or as part of a networked system and will be responsible for the quality of the information. The amount of time spent on this type of work will depend on the job. All staff has a duty to ensure that all relevant manual and computerised data is collected and entered in an accurate and timely manner, ensuring all activity undertaken is accounted for. Working in Teams Play an active role as members of interdisciplinary teams Collaborate and communicate effectively with nurses and the wider MDT, including professionals from social care and education. Support in the supervision of Nursing Associate students on placement with the CCNT. Improving Safety and Quality of Care Improve the quality of care by contributing to the continuous monitoring of CYP and families experience of care. Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of CYP first. To participate in effective and comprehensive audits and measure against benchmarked targets set within the framework. Be aware of and maintain the medical equipment database to ensure all equipment is serviced as per SWFT policy. To contribute to policy reviews. Contributing to Integrated Care Contribute to the provision of care for CYP, including those with complex needs Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a CYP and their family Understand responsibilities in relation to communication and collaboration.
