Job Title-HCA or Nurse Associate for the Enhanced Care Team, Bournemouth East Primary Care Network Contract Type-Permanent Hours:up to 25 hours per week. Full or part time. Working pattern negotiable. Main duties and responsibilities (this list is not exhaustive): Undertake Frailty Assessments using the Dorset Care Plan/PCSP Signposting to other services/social prescribers/voluntary sector as appropriate. Conduct long-term condition housebound reviews Provide phlebotomy service Perform basic patient observations, wheelchair assessments, pain assessments, ECGs and other health checks Maintain accurate clinical records via SystmOne To work within agreed policies and procedures to promote safe clinical practice Training will be provided in these areas, depending on experience. We are also able to support NVQ qualification.
