The appointee will be one of seven consultants in the Diabetes & Endocrinology department, to participate in the General Internal Medical in-patient duties of the department. The ward-based model of medical in-patient service is provided by Acute, Elderly and General/ Subspecialty teams, in a ward based model. Apart from Acute Medical ward areas and Medicine for the Elderly wards, base wards are assigned to Cardiology, Stroke, Respiratory, Gastroenterology, and Diabetes & Endocrinology. The appointee will be expected to work with the other Consultants in the department of Diabetes & Endocrinology to deliver a comprehensive general internal medical service for in-patient care for the Diabetes & Endocrinology base ward. At present this is a 30 bedded ward, with a mix of complex medical patients as well as subspecialty (mostly diabetes) patients. The ward duty is shared with other consultants in the department of Diabetes & Endocrinology who rotate to in-patient duties on a regular basis. Together with the subspecialty consultant, the post holder is responsible for daily review of patients including those newly admitted/ transferred to the ward, supervision of junior medical staff and nursing staff, and running multidisciplinary Board rounds and daily review of patients to ensure safe, effective management and early discharge. Complex endocrine and diabetes in-patients (such as diabetic foot patients and complex type 1 diabetics) are admitted to the Diabetes & Endocrine base ward as part of the Needs Based Care model. The post holder will be expected to manage these complex specialty patients in close coordination with the specialty consultant sharing the ward, and the specialist nurses. Medicine operates a seven day model of care, and there is consultant led weekend ward round in all base wards, which is supported by one junior member of staff. The post holder is expected to participate in weekend ward rounds on a 1:8 rota. The post holder is expected to participate in the overnight on-call B rota (see below) in a 1:13 rota. The B rota is a low intensity low frequency on-call from home providing telephone guidance and support, but available to be called back if required. There is requirement to undertake a post-discharge general medical out-patient clinic to see follow-up patients. There may also be an optional specialist activity, if desired. Applicants must be fully registered on the UK General Medical Councils Specialist Register and have MRCP. Specialist Registrars may apply if within 6 months of CCT. Candidates unable for personal reasons to work full-time are also invited to apply.
