• Occupational Therapists
• Physiotherapists
• Speech and Language Therapists
• Dietitians
• Registered Nurses
• Support Workers across Occupational Therapy, Rehabilitation, Physiotherapy, Dietetics & Nursing

To work alongside stroke therapy team in both the acute and community toprovidea flexible range of integrated, therapeutic services for the localstrokepopulation; toidentifyand review individual clients’ needs, ensuring that they are receiving an effective,equitableand personalisedservice.To coordinatetimelyclient care by supporting and encouraging integrated working across multiple services and organisations, engaging with a broad range of health and social care providers. To contribute towards and support the development of quality indicators and metrics to evaluate the impact of the role in supporting service users with complex stroke.
