Slough OPMH has an exciting opportunity for aCommunity PsychiatricNurse who wishes to join our dynamic Home Treatment Crisis team. A flexible working shift patternis just one of the attractions this opportunity will provide to you. You will provide high quality treatments and will work with the clinical team to take responsibility for the assessment and implementation of intensive care and treatment people with mental health needs/dementiain the community.


You must have specialist knowledge of the care of older people with mental health problems and mental health crises. You will be required to travel within the community for the performance of your duties.


We are a friendly, dynamic and supportive team and you will have regular access to supervision and peer support in your role. • Participating as a member of a multi-disciplinary team managing a caseload of patients
• To undertake assessments of patients, involving theircarersand family members in the patient's care and care plans.
• To be responsible forthe formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
• The post holder will direct and supervise Support Workers and/or students.
• Post holder will provide advice to other members of the service and agencies on the needs of service users.
• To provide a fast and flexible response to care needs, responding to referrals in a timely mannerand providing intensive treatment (up to four times daily).
• To initiateand carry out the agreed treatment plan

The "must haves" for you to be considered for this role:
• Have the relevant professional qualification for a Mental Health Nurse
• Able to work flexibly on a shift pattern to cover CRHTT Service as required and the ability to travel independently between locations to fulfil the requirements of the position.
• Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs
• Good understanding of the relevant legislation including the Mental Capacity Act / Mental Health Act
• The ability to assess risk in both routine and urgent situations
