This post will contribute to tackling inequalities in health through promoting and supporting people to stop smoking. It focuses on engaging with people in communities and offering them practical support to change their behaviour to achieve their own choices and goals. The post holder will support the specialist stop smoking advisors delivering sessions for people stopping smoking. Specifically the post holder will: Engage with people in local communities which have identified health inequalities. Communicate with people about health and health improvement. Work with people to change their behaviour to improve their health. Manage and organise their time and activities to give smokers support to stop smoking, this may be face to face in group settings or one to ones and over the telephone. Engage with people in local areas which have identified health inequalities. Identify and make contact with people in the local community to work with. Work with existing groups and support the development of new groups in order to identify and engage with individuals. Develop and maintain relationships with people who are most at risk of poor health outcomes. Promote the equality and value the diversity of individuals. Build up and maintain knowledge of contacts and agencies within the community. Communicate with individuals about health and health improvement. Provide information to people about stopping smoking and health related issues; Enable people to develop their knowledge and skills about health and well-being. Signpost individuals to other agencies for information, support and resources as necessary. Enable people to change their behaviour to improve their health. Help people to understand how smoking affects their health. Help people to make a personal health plan which includes changes they want to make. Support people in achieving their personal health plan by offering encouragement and motivation on a weekly basis. Provide weekly support for people to maintain their behaviour change. Manage and organise own time and activities to support individuals in the community. Plan own time and activities to meet the reasonable needs of individuals in the community. Respond effectively to referrals within stated timescales self-referrals from the individuals, and those from colleagues /other workers/GPs. Keep adequate electronic records using Quit Manager of the work undertaken as agreed with line manager. Alert specialist advisors or line manager to any issues in work (including concerns about individuals or work in the community) and maintain confidentiality. Actively contribute to ways in which local services can be improved to improve the health and well-being of the local community/barriers that individuals are experiencing in changing their behaviour. Contribute to the development of the Specialist stop smoking service targets and any plans that may be developed by attending meetings as appropriate with co workers and sharing information. Take an active part in developing own knowledge and skills. Seek advice and support as and when appropriate. Monitor and maintain health, safety and security of self and others. Adhere to organisational policies and procedures. To participate in an annual performance and personal development review. To undertake any other duties consistent with the grade. To maintain good working relationships with all levels of staff within the Acute Care Business Unit to ensure these administrative duties are carried out efficiently and effectively in order to meet the needs of the service.
