Description
The Personal Advisor for the Leaving Care Service (SO2) plays a crucial role in supporting young individuals as they transition out of care and into independent living. This position is essential in ensuring that care leavers receive the necessary guidance, support, and resources to thrive in their new environments. As a Personal Advisor, you will work with a diverse group of young people, helping them navigate the challenges that come with leaving care, including housing, education, employment, and personal development. This role not only requires empathy and understanding but also a strong commitment to empowering young individuals to achieve their goals and realize their potential. You will be responsible for creating and implementing personalized support plans that address the unique needs of each care leaver, while also advocating for their rights and best interests within the community. By fostering relationships and building trust with young people, you will play a pivotal role in their journey toward independence, acting as a mentor and a reliable source of support. Responsibilities
Provide one-on-one support and guidance to care leavers as they transition to independent living.
Develop and implement personalized support plans that address individual needs and goals.
Assist in accessing education, employment, and training opportunities for young people.
Help young people secure stable housing and manage their finances effectively.
Advocate for the rights and needs of care leavers within the local community and relevant organizations.
Maintain accurate records and documentation of support provided to each individual.
Monitor progress and adjust support strategies as necessary to ensure successful outcomes for care leavers.
Requirements
A relevant qualification in social work, youth work, or a related field would be advantageous.
Experience working with young people, particularly those in care or leaving care.
Strong understanding of the leaving care process and the challenges faced by young individuals.
Excellent communication and interpersonal skills, with the ability to build rapport and trust.
Demonstrated ability to develop and implement individual support plans tailored to young people's needs.
Knowledge of local resources and services available to support care leavers in their transition to independence.
A compassionate and patient approach, with a commitment to empowering young people to achieve their potential.

Pay Per Hour: £19.95
