Crynodeb
Deliver comprehensive mental and physical health assessment, support care planning and implement evidence based interventions. Work closely with the ANP lead, PCN Business Manager, and the wider Health Inclusion Team to ensure high quality, inclusive care delivery. Collaborate with services across health, housing, safeguarding, addiction and voluntary sectors to develop tailored integrated care plans. Contribute to the ongoing development, monitoring and evaluation of the Health Inclusion Team's work. Support individuals to access healthcare and community services, advocating on their behalf where appropriate.
