The Home First service brings together nurses,therapists,healthand therapy assistants, to work with the acute Multi-Disciplinary Team & Transfer of Care Hubs to promote an efficient, effective discharge from hospital to the patient’s own home.


Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission. Home First will, whererequired, provide acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

We are looking for aSenior Nurseto join us, so ifyou’rean ambitious, proactive, and experienced nurse looking to take a lead role in a team that providescutting-edge, excellent community care,we’dlove to hear from you!


As a Senior Nurse you will be providing holistic assessments, planning, implementing, and evaluating treatment care plans, alongside reviewing care provision.


In this role you willbe responsible forstaff management, supervision, and development, including shift coordination, allocation of visits, and the performance development reviews of members of the team.


You will use your clinicalexpertiseand problem-solving skills to support patients and the team. This involves completing clinical assessment, planning and the implementing care for patients including medication management, wound care, catheter care andidentifyingdeteriorating patients.  


As a member of the team, you will receive support from members of the multidisciplinary team andparticipatein the in-service learning programme. While developing your own specialist areas of practice, relevant to the team, you will supervise and support the development of colleagues, including those outside of your professional group and pre-registration students. 


You will play an active role in the development of the service through reflective practice, clinical supervision, and quality improvement initiatives. This is a fantastic environment in which to develop your leadership skills through formal and supported training, using these toassistin shaping future clinical practice of the team.
