Under the delegation and supervision of the registered healthcare practitioner, the health care support worker will carry out all of the delegated duties listed in the main responsibilities section below independently within the health care team. The post holder will provide the fundamentals of care and extended skills to patients in the outpatient department to ensure high standards are maintained in a safe environment including patient safety and to make sure that the care experience is positive by treating all patients, family, relatives and colleagues with respect, dignity and courtesy in accordance with Trust values and policies.
• Support patients when they are attending their outpatient appointments.
• Prepare Notes and ensure patient requests are fulfilled or actioned
• Work with the other team members to ensure a safe and collaborative environment is adhered to
• Keep the environment well stocked and complete any audits requested.
• If necessary, take observations in line with the recommendations of the Care Certificate.
• Ensure patient care is given in line with the Trust expectations and patients individual needs are met.
• Ordering stock for the unit and keeping records up to date.
• Signposting patients to the correct service and working as a team with other staff members.
• Ensure all mandatory training is kept up to date in line with Trust expectations and patient safety needs.
• Willing to work in other areas of the Trust as and when required.

Individuals in a band 3 will agree with their manager to undertake additional skills / tasks relevant to their area of work and in addition to those roles in the Band 2 job description. The individual Band 3 must attend all relevant training and have a completed Trust competency before practising unsupervised. Once assessed competent the individual band 3 must not perform a skill / task unless it has been delegated to them by a registered practitioner and consent gained from the patient on each occasion, these may include;

Vital observations
These will form part of a Band 3 Skills Matrix.
Will have the knowledge and skills to provide the fundamental care needs for individual patients under the direction and supervision of the registered practitioner, the following can be undertaken in the band 3
training period, following the GWH Care Certificate Standards of Care.
Work with others towards achieving shared goals, learning from mistakes and being open to the opinions of others including patients, receiving and giving constructive feedback.
Be open and respond constructively to change, coping with uncertainty and taking the lead in supporting others in the team as appropriate.
Make best use of resources including time, to achieve agreed goals for service delivery, taking responsibility for own actions and self-development
