Address the needs of individuals to bring about improvement in the health and wellbeing of patients through the delivery of evidence-based care Manages the ward on a regular basis, recognising and responding effectively to the changing nature of the workload. Model excellent practice through effective assessment, planning, implementation and evaluation of care. Articulate and discuss source of evidence on which care is based. Articulate the decision making process underlying the management of own and others time and the patient workload. Initiate and develop plans for health promotion with individuals involving parents/carers and multidisciplinary teams. Demonstrate involvement with the multi-disciplinary team. Has knowledge of the needs of all Trauma and Orthopaedic patients and is able to communicate their needs to other members of the multi-disciplinary an multi-agency teams. Liaises with specialist nurses and other disciplines at spoke hospitals to ensure high standards of care across the Region. Recognition of organisational and clinical needs that may affect the area in which they work. Initiates and maintains effective communication with others about the patients in their care. Negotiate sensitively with patients, relatives and carers regarding care and discharge planning. Behave in a consistent manner with patients, relatives and theircarers, the multi-disciplinary team and the public. Liaises with ward manager to manage sickness and staffing. Liaise with site managers, bed managers and matrons to and multidisciplinary team regarding admissions, discharges and flow of patients between departments Demonstrate use of IT skills to complete details of admission/discharge of patients using a computerised record system Write precise, accurate and timely nursing records, reports and statements. Listen actively and respond appropriately to information given. Leads shift handovers ensuring relevant detail is communicated to the team Recognise when own and others behaviour is not acceptable and take action to address this effectively. Provides constructive feedback to individuals and the team. Responsible for maintaining own professional development and portfolio. Attend statutory and mandatory training days as required by UHS. Act as a mentor to all staff offering guidance, support and education (formal and informal). Undertake further competence based training to enable development of role. Participate in own appraisal, the appraisal of others and support staff in the setting of their Personal Development Plans. Works towards achieving the trust objectives as laid out in the Nursing and Midwifery Strategy. Facilitate and support the clinical educator and support the development of other staff to undertake a link nurse role as designated by the Ward Manager. Lead and delegate mandatory and other audit as required by the Trust In conjunction with other clinical staff, sets, monitors and evaluates standards of care for patients in all areas. Facilitate the cascading of information from other sources in the absence of the Ward Manager Awareness of organisational structure and the function of the Directorate within the organisation. Participation in and promote others participation in Trust wide forums as designated by the Sister Prioritise care and delegate duties to others asappropriate ensuring they are supervised and mentored taking into account knowledge, skills and ability of others. Maintain and raise awareness in others of the efficient use of all resources within the directorate Develops and sustains effective partnership working with individuals, groups, communities and other agencies to process admissions/transfers and coordinate discharges in accordance with UHS policy. Liaise effectively with colleagues, the multi-disciplinary team, the Wardmanager and other departments to allow the timely management of patient throughput.
