Start date: Tuesday 1st April 2025 Full time Band 7-8a inner You will be working solely at ICMC but employed by N2 Islington PCN Site ICMC The practice pharmacist must: provide exemplary care provide support and advice to the other members of the MDT advise the practice of the safe management of medicines assess the wellbeing of patients in regard to their medication have competency across a range of pharmaceutical subjects supported by professional and clinical knowledge contribute to patient safety culture help lead person centred medicines optimisation promote patient involvement in their care participate in regular appraisals and attend training and development activities maintain a non-judgemental attitude uphold patient confidentiality plan and manage and monitor and review general care plans for patients with chronic conditions Accountable to: GP Partners and Practice Manager
